Spirit of the Game Award
The Spirit of the Game award is the highest award in our program. The award is given to a member of the varsity team that embodies the following:
"The game relies on the integrity of the individual to show consideration for other players and to abide by the rules. To conduct themselves in a disciplined manner, demonstrating courtesy and sportsmanship, irrespective of how competitive they may be."
Integrity - Adherence to moral and ethical principles, soundness of moral character and honesty.

These forms MUST to be completed by Wednesday March 23rd to participate in the 1st practice Monday April 4th
- Athletic Registration Form - Includes the Parent/Athlete Guidebook Agreement, and the Release Waiver, and Indemnification of Liability, Emergency Treatment/Participation Clearance, and Student's Health Information. Must be completed and turned in with the Activity Fee ($65.00) for each sport participated in (family cap of $325.00). For information only. Please use original form available in both Athletic Office and High School Office.
- WIAA Physical Card - WIAA Green Physical Card (Doctor and Parent Signatures required) A physical date of April 1, 2014 or after is current. If you have not had a physical since April 1, 2013, you will need to get one prior to participating.
- WIAA Alternate Card - If the date of your last physical is between April 1, 2013 and March 31, 2014, you will need to submit a completed Tan WIAA Alternate Year Card. This requires a parent signature only.
- Parent/Athlete Concussion Acknowledgement Form - At the start of every season, a new concussion form will need to be turned in to the coach prior to participation in a practice or contest. The form requires the signature of both the athlete and a parent.
- Transportation Form - Transportation to the golf courses for practice or home meets will be the responsibility of the athlete. This form gives authorization for a coach, another parent, or fellow student to provide transportation to your son to the golf courses.

Transportation to and from away meets will be provided by the school. If your son is not traveling back with the team a Travel Release From MUST be completed, signed by the parent/guardian, and returned to the coach before a student is cleared to travel with his parent/guardian, and returned to the coach after the contest. Only a parent/guardian may transport their son.
