Sports physicals are good for up to two years, depending on the date. Currently, if you have a physical dated April 1, 2024 or later, you are good for the 2024-25 school year. If you had a physical between April 1, 2023 - March 31, 2024 and it is on file in the Skyward Portfolio area, you will need to complete and upload the Alternate Year Card.
Alternate year cards are needed on non physical years. If you had a physical last school year and it is on file in Skyward, parents/guardians should complete the alternate card and upload it as a part of Athletics registration in Skyward. Please note that it does ask for the date of the most current physical.
If the most recent physical on file in Skyward is dated prior to April 1, 2023, it is expired and no longer valid. This means you will need a new physical completed prior to your 2024-25 sports season starting.
Please note that all forms require a parent signature and date at the bottom before uploading to Skyward as part of the registration steps. Athletic registration for the 2024-25 school year will open mid-July.
All incoming freshmen will need a WIAA physician signed physical form. Each subsequent year a physical or alternate physical card (filled out by a guardian) is required to be uploaded during the athletics registration process. Only one card will be accepted for upload - so please be sure to upload the correct card. Parents can check which physical card their child needs each year by going into Skyward into the left hand bar which reads: “Portfolios and Reports”. Clicking on this will show a listing of uploaded items including previous years' physical cards. WIAA Preparticipation Physical Evaluation Clearance Form (Parent AND Doctor Signatures required) for a physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year.

This form MUST be completed, signed by the parent/guardian, and returned to the coach before a student is cleared to travel with his parent/guardian, and returned to the coach after the contest.
